1. Is there any reference design for PCB antenna?
2. What will be the drill hole and plating of stitching via?
3. what will be the distance between feed point to the antenna?
4. Is there any changes in schematic for external power amplifier?

In this document you could find Guidelines for designing printed antennas for BLE applications on single layer and multi-layer PCB stack-ups. Also, you are able to use take an antenna directly from available dxf formats but you should match it according to your PCB. You can find the available dxf formats in this linkAN-B-027: Designing Printed Antennas: DXF files. Regarding the reference design application, you can check the Range Extender.

Our board is 0.8 mm two layer and width is 15 mm only. Your recommended antennas cannot suite for my board. can you please recommend suitable for our design? or can you provide any link for calculating antenna designing?
